On-TRAK is a specialist alcohol service that operates in both Dewsbury and Huddersfield. As an Alcohol Shared Care Nurse, you will participate as a member of a multi disciplinary team delivering treatment services to adults affected by harmful or dependent drinking. You will be responsible for delivering specialist services in Shared Care practices including providing supported community detoxification and delivering psychosocial interventions in line with Models of Care for Alcohol (MoCAM) and Nice guidelines. You will also be part of a team of nurses delivering a blood borne virus service to On-TRAK service users.
You must have a minimum 12 months post qualification nursing experience, experience of information and advice giving in a health or social care setting and experience of working with alcohol misusers. You must also have a Nursing Qualification (RN or RMNH) and current registration with NMC, with evidence of continued update to practice.
